The Occupational Therapy Department Students who are taking the course OT401 CLINICAL EDUCATION

The Healthcare Management Department Students who are taking the course HS403 EDUCATION

The Nutrition and Dietetics Department Students who are taking the course ND401 EDUCATION AT WORKPLACE I

The above students will go to hospitals for the pratical hours of these courses. The practical hours will have an impact for course grades. It is an obligation to attend the practical hours at hospitals.

The important dates and notes for this procedure:

1. Every student has to pay the amount of registration fee (200 TL) till the 05.10.17 until 5 pm. (you must check the guideline below for bank accounts- please click on it) and bring the bank receipt to the Assoc. Prof. Dr. Ayşe Seyer till 06.10.17 10 am. 

2. Between the dates of 2.10.17- 5.10.17 you should visit your advisors office to take the STUDENT APPLICATION FORM and fill it.

3. Every student should take a MEDICAL REPORT (You can take it from your country or from TRNC Dr. Burhan Nalbantoğlu Hospital, TRNC Girne Akçiçek Hospital or any full-fledged university hospital). If you are going to take it from TRNC you should pay 135 TL for this. also the medical report should include the medical tests and subjects which are represented in Student Registration Terms for foreign student. The medical reports will not be accepted after the date of 05.10.17. You can bring the reports to Assoc. Prof. Dr. Ayşe SEYER ÇAĞATAN.

ANY STUDENTS WHO DO NOT HAVE BANK RECEIPT, MEDICAL REPORT and STUDENT APPLICATION FORM WILL NOT BE ACCEPTED FOR THE INTERNSHIP!!!

Öğrencilerin DİKKATİNE!

Ergoterapi Bölümünde OT401 CLINICAL EDUCATION dersini alan öğrenciler

Sağlık Yönetimi Bölümünde HS403 EDUCATION dersini alan öğrenciler

Beslenme ve Diyetetik Bölümünde ND401 EDUCATION AT WORKPLACE I dersini alan öğrenciler

Yukarıda belirtilen dersleri alan öğrenciler derslerin pratik saatleri için hastanelerde uygulamaya çıkacaklardır. Uygulama dersleri dersin notuna etki edeceğinden dersleri alan öğrencilerin bu uygulamalara katılması ZORUNLUDUR!

Önemli Tarihler ve Unsurlar:

1. Uygulamaya çıkacak tüm öğrenciler Sağlık Bakanlığı tarafından talep edilen 200 TL yi aşağıdaki linke tıklayarak öğrenebileceğiniz hesap numaralarına yatırarak dekontları en geç 05.10.17 mesai saati bitimine kadar Yrd. Doç. Dr. Ayşe Seyer Çağatan a teslim etmeleri gerekmektedir.

2. Uygulamaya başlayabilmek için gerekli olan STAJ BAŞVURU FORMLARINI 2.10.17-5.10.17 tarihleri arasında danışmanınızdan alıp doldurmalısınız.

3. Sağlık Bakanlığı tarafından istenilen SAĞLIK RAPORLARINI 5.10.17 tarihine kadar Yrd. Doç. Dr. Ayşe SEYER ÇAĞATAN a teslim etmelisiniz. Türkiye de bulunan öğrenciler herhangi bir devlet hastanesinden veya tam teşekküllü üniversite hastanesinden raporları alabilirler. Yine TC vatandaşı olup şu anda KKTC de olan öğrencilerimiz veya Türkiye den alamayan öğrencilerimiz KKTC Dr. Burhan Nalbantoğlu Hastanesinden veya Girne Akçiçek Hastanesinden veya tam teşekküllü üniversite hastanesinden 135 TL ödeyerek alabilirler. Sağlık raporları Öğrenci Kayıt Şartlarında belirtilen tüm tıbbi test ve kontrolleri içermelidir.
